How you’ll make an impact in this role

• Lead personalized care journeys by implementing and monitoring tailored care plans that directly improve patient recovery and safety.

• Coordinate the multidisciplinary team as the central point of contact, ensuring seamless communication between doctors, specialists, and families.

• Champion clinical accuracy by assessing patient conditions in real-time and documenting their progress to ensure a high-quality, data-driven care experience.

• Empower patient transitions through strategic discharge planning, ensuring every individual is prepared for a safe and confident return home.

What minimum requirements you’ll need

Licensure / Certification / Registration:

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Licensed Registered Nurse credentialed from the Texas Board of Nursing required.

Education:

• Diploma from an accredited school/college of nursing required OR a bachelors level degree not in nursing, an active RN License, and enrolled in an Alternate Entry Masters of Science in Nursing Program with a minimum of 1 year in the program completed.

• 6 month women's health and family practice experience
